Volunteer Firefighter Application

Township of Champlain Fire Department is looking for Volunteer Firefighters. Being a Volunteer Firefighter can be a very rewarding experience as you contribute to the safety of the entire community.

Volunteer Questionnaire

All applicants must have a Grade 12 or approved equivalent, have more then 18 years of age and must be physically able to perform the essential duties of a Volunteer Firefighter.

 

Are you legally entitled to work in Canada?
 
Have you ever been convicted of a criminal offence for which a pardon has not been granted?
 
Do you have a valid Ontario class “G” Driver’s Licence or higher?
 
Do you have no more than 3 minor moving violations in the last 3 years, no driver's licence suspensions in the last 3 years and no criminal code convictions?
 
Do you have any of these certificate -check as many as apply
 
This position requires you to work in confined space and high places such as roofs, balconies, ladders etc. Are you able to meet these requirements of the position?
 
Do you have vision of at least 20/40 in each eye without corrective lenses, normal depth perception and color vision?
 
Indicate your highest level of education achieved
 
Have you taken any other courses that would be applicable to this position? (IE: Pre-Fire Service, Police Foundations, EMT)
